Guide
Emergency card and preparing for doctor visits
With a rare disease, the emergency team may not always know the condition. Carrying key information with you and preparing for appointments makes things easier.
In an emergency
- In an emergency in Türkiye, call 112.
- At the emergency department, state the diagnosis and the centre that follows you, and show this card.
- Keep your doctor's emergency letter or instructions and the medication list with you.
- If the team asks, give them the phone number of the doctor who follows you.
Prepare your card
Fill in what you know; empty fields do not appear on the card.

Guidelines for emergency doctors
Orphanet publishes guidelines for emergency doctors for some rare diseases. These guidelines are written for doctors. Ask your doctor whether there is one for your condition and what to write on your card. See on Orphanet
Preparing for a doctor visit
- Write your questions down before the visit; put the three most important first.
- Bring the names and doses of all medicines.
- In Türkiye, you can see your reports and test results in e-Nabız.
- If possible, bring someone with you; one of you can take notes.
- Do not hesitate to ask about any word you do not understand.
Questions you can ask your doctor
- Is the diagnosis confirmed? Which test confirmed it?
- How does this disease affect our family? Can we get genetic counselling?
- Which symptoms mean we should go to the emergency department right away?
- Can you give us written emergency instructions or a letter?
- What treatments are there? How can we access them in Türkiye?
- When is the next check-up? Which tests will be done?
- Is there anything we should know for school, nursery or work?
