How does it appear on your report?

  • PT, PTZ (prothrombin time): The time it takes blood to clot with certain factors. Under the Turkish Ministry of Health unit standard, it is reported in seconds (s).
  • INR: A standardized ratio of PT that removes differences between analyzers. It has no unit.
  • aPTT (aPTZ): A separate clotting test. For details, see the elevated aPTT page.

According to the Turkish Biochemical Society's 2020 coagulation guideline, INR is calculated with a reagent whose international sensitivity index (ISI) is known. This makes INR results from different laboratories comparable. According to the same guideline, PT is sensitive to deficiencies of factors I, II, V, VII and X.

Why does it appear red in e-Nabız? The "Tahlillerim" (My Tests) screen of e-Nabız (Türkiye's national health record app) shows results with reference values and colors out-of-range values. The reference range is based on people not taking blood thinners. If you take warfarin, an INR that is on target (for example 2.5) will still appear in the "high" color. This alone does not mean there is a problem.

Reference range or target range? The range written on the report is the value for healthy people. The warfarin target, on the other hand, is a treatment target and is usually not written on the report. Under the Ministry procedure, decision limits are marked with an asterisk (*); a critical value is also defined for INR.

The reference range covers 95% of healthy people; about 1 in 20 healthy people may fall slightly outside the range. A single borderline value does not by itself indicate a clotting disorder.

What does it mean if it is high?

Causes of prolonged PT or high INR, from common to serious:

  • Expected drug effect: Warfarin reduces vitamin K-dependent factors. Raising INR into the target range is the aim of treatment.
  • Sample problem: According to the Turkish Biochemical Society, times are prolonged if the citrate tube is underfilled. In small tubes, filling below 90% can give a falsely high INR.
  • Very high hematocrit: If hematocrit is above 55%, the citrate ratio in the tube is disturbed and the time may be prolonged.
  • Low vitamin K: Long-term antibiotic use and poor nutrition can reduce vitamin K and prolong PT.
  • Newer blood thinners: According to StatPearls, apixaban, rivaroxaban, edoxaban and dabigatran can prolong PT.
  • Liver disease: Most clotting factors are made in the liver. When liver function is impaired, PT is prolonged.
  • Disseminated intravascular coagulation (DIC): In severe illness the factors are used up. It is a serious condition diagnosed in hospital.

If your INR is above target while taking warfarin, the risk of bleeding increases. Do not change or skip the dose yourself. Contact your doctor or the unit following you the same day.

What does it mean if it is low?

  • Below target while taking warfarin: The drug's protection against clots may be insufficient. Your doctor may ask about how you take the drug, any new medications and your diet.
  • Prolonged tourniquet: According to the Ministry of Health's 2024 guide, a tourniquet left on for a long time can shorten PT.
  • Pregnancy: Some clotting factors increase; PT may shorten slightly.

In someone not taking blood thinners, your doctor evaluates a low INR together with other tests.

Read together

  • Only PT prolonged, aPTT normal: Factor VII affects only PT. Conditions that lower this factor, such as warfarin or low vitamin K, come to mind.
  • PT and aPTT both prolonged: Factors in the common pathway (I, II, V, X) may be affected. Liver disease, DIC or severe vitamin K deficiency is considered.
  • High INR + abnormal liver tests: The liver's production of clotting factors may be reduced. The result is evaluated together with liver tests.
  • Newer blood thinner + INR: According to the EHRA 2021 guide, these drugs are not monitored with routine clotting tests. Fingertip INR devices developed for warfarin also do not accurately show the effect of these drugs.
  • Signs of bleeding + low platelets: The platelet count in the complete blood count is also evaluated.

Preparation before the test

  • Time and fasting: The Turkish Biochemical Society guideline recommends 07:00–09:00 in the morning after 8–12 hours of fasting.
  • Rest and smoking: Rest seated for 10–15 minutes before the blood draw. If you smoke, give blood at least 30 minutes after your last cigarette.
  • Same time for follow-up tests: During treatment follow-up, it is recommended that blood be drawn at the same time of day each time.
  • Report your medications: Tell the blood-draw team which blood thinner you use, the time of your last dose and any medications you have recently started.
  • Tube: The clotting test is drawn into a light blue-top citrate tube. The tube must be filled to the line on it; underfilled or overfilled tubes are rejected.
  • Waiting time: According to the guideline, a PT sample can be analyzed within 24 hours.

Which doctor, which test?

  • Cardiology: If you take a blood thinner for atrial fibrillation or a heart valve, sets the target INR.
  • Hematology: If you have an unexplained prolonged PT or a bleeding tendency even though you do not take a blood thinner.
  • Gastroenterology: When liver disease is suspected.
  • Family physician or Internal Medicine: Regular INR follow-up and review of drug interactions.
  • Additional tests: aPTT, fibrinogen, complete blood count (platelets), liver tests and factor levels the doctor considers necessary.

In Türkiye

  • Critical value: Under the Ministry of Health procedure, an INR of 5.0 and above is a critical (panic) value. The laboratory reports this result to the doctor as soon as possible.
  • Legal basis: The Medical Laboratory Services Regulation dated 24.10.2025 requires every laboratory to define critical values and set up an alert system.
  • National pre-analytical guideline: The Turkish Biochemical Society's 2020 guideline recommends a 3.2% sodium citrate tube and a 9:1 blood-to-citrate ratio.
  • Tracking with a chart: In e-Nabız you can see your INR results over time as a chart. This is useful when discussing with your doctor how much of the time your INR stays on target.
  • Hemolyzed sample: According to the Ministry guide, hemolysis affects clotting tests and the laboratory may reject the sample.

When should you seek urgent care?

If you take warfarin or another blood thinner:

  • If you have black, tar-colored stools, vomit blood or have visible blood in your urine, call 112 (the emergency number in Türkiye).
  • Go to the emergency department immediately for a nosebleed that will not stop, a sudden severe headache or after a blow to the head.
  • If you have difficulty speaking, facial drooping, or loss of strength in an arm or leg, call 112.
  • If you have sudden shortness of breath, chest pain or painful swelling in one leg, call 112.
  • If the laboratory has called you about a critical value (INR 5.0 and above), contact a doctor the same day. If there is bleeding, go to the emergency department without waiting.

Are PT and INR the same thing?

They come from the same test but are not the same thing. PT is the clotting time measured in seconds. INR is the ratio that makes this time comparable between laboratories. INR is used for warfarin monitoring.

I take apixaban or rivaroxaban. Do I need to have my INR checked?

My INR came back high. What should I do?
